Add a “How did you hear about us” question to your Shopify store

Today, I was working on adding the question “How did you hear about us” to a client’s Shopify store. Fortunately, Shopify has a great document with how-to-steps “Ask how customers heard about your store.” This Shopify document walks you through how to add a snippet to your code base, include the snippet in your cart, update configurations, make the form a required element, and add this customer response to your new order emails.

Customize the options customers can choose

As of June 2023, the only step I found a little challenging in this documentation was finding where to Set your options. In case this stumps you too, here are the two ways to set these form options.

Set options from Theme Settings

First, click the the “Customize” button, then the paintbrush button to open Theme Settings. Then toggle “Hear about us” to reveal the “Form Options.”

Set options from Cart Pages

First, click the the “Customize” button, then select “Cart” from the page drop down located at the top of the screen. Next, in the left menu toggle open “Theme Settings.” You’ll find the field, “Form Options” where you can customize the options. Don’t forget to comma separate your options.
